Patient Navigator

Patient Navigators are ward-based nurses who steer patients through the healthcare system – from tertiary to community, primary and home care.

In the wards, they collaborate with our multidisciplinary team, patients and their family members to develop appropriate care plans and coordinate recommended referrals to appropriate community and social agencies.​
