Heart Failure

Heart Failure specialty care nurses collaborate with heart failure cardiologists, cardiothoracic surgeons, nurses, pharmacists, dietitians, physiotherapists and medical social workers to provide comprehensive care to patients diagnosed with heart failure.

Heart Failure specialty care nursing services:

• Support and provide consultation on heart failure care to healthcare team at the inpatient and outpatient clinical services
• Provide health education to enable patients in managing heart failure condition
• Provide follow-up care at the outpatient clinic to patients requiring post discharge monitoring and titration of heart failure medications
• Provide post discharge telephone follow-up
