Adult Congenital Heart Disease / Pulmonary Hypertension

The clinical nurse specialists in Adult Congenital Heart Disease/ Pulmonary Hypertension (ACHD/ PH) provide the following services:

Patient Education: 

• Explain the underlying congenital heart and medical condition to the patient, family and caregiver, and provide individualised health education.
• Provide brief counseling on specific social or behavior issues e.g. avoid tattooing, body piercing (without explaining why may lend connotations on stereotyping), dental prophylaxis antibiotic.

 Nurse Consultation:

• Personalised management on ACHD/ PH and other related issues.
• Telephone consultation with patients and their families.

 Coordination of patient care:

• Collaborates with the multidiscplinary team to carry out treatment plans.
• Facilities the transition of young adult patients from paediatric to adult cardiology setting.
