Care and Counselling

We offer patients a safe and private environment to explore and learn to cope with the challenges arising from illnesses or disabilities. 

Assessment of care needs

  • Counsel and educate patients and families on the available community resources
  • Co-ordination of care arrangements identified by patients and families (e.g. day care/day rehabilitation applications)
  • Monitor patient’s coping at home post-discharge

Psycho-social support

  • Assessment of coping abilities and support networks
  • Discuss ways to cope and manage issues arising from illnesses such as anxiety, grief and loss, bereavement and other emotional distress
  • Counselling
