Advance Care Planning (ACP) is an ongoing discussion with your loved ones on your preferred healthcare treatments. We facilitate discussions with patients to explore on the following issues:

Your understanding of your medical condition and its potential complications
Your concept of living well
Your experiences in living well
Your values and goals in life
Your goals in medical treatments
Identify a nominated healthcare spokesperson healthcare proxy who will help make decisions on your behalf when you are not able to speak for yourself
During the discussion, it is encouraged to have your nominated healthcare spokesperson present so that he/she is aware of your treatment preferences
Communicate your preferences to your loved ones so that they can honor your wishes
