If you wish to apply for a medical report, we require you to consent to releasing the medical information. If you are below 21 years old, you are required to obtain the consent of your parents or legal guardian. If the person you are applying the medical report for has passed away, a copy of the death certificate and the consent from the deceased's spouse are required.

Note: Due to the current COVID-19 situation, a longer processing time for medical reports is expected during this period. Apologies for any inconvenience caused.
You need to bring along your NRIC when collecting your medical report.
If you are collecting the report on someone else's behalf, you need to produce your NRIC and get an authorisation letter from the patient.
If the patient is deceased, a copy of the patient's death certificate, supporting documents on proof of relationship and the consent of the patient's spouse/Grant of Administration are required.
If the patient is a minor, the application is to be made by the patient's parents or legal guardian. A copy of the patient's birth certificate is required.Visit us at Medical Reports Unit (4E, Level 4), National Heart Centre Singapore. The operation hours are as follows:
Monday to Friday : 8.00am - 5.00pm Saturday : 8.30am - 12.00pm* Sunday and Public Holidays : Closed*Due to the enhanced COVID-19 national measures, our Medical Reports Unit will be temporarily closed on Saturdays.

Please quote the name and NRIC of the patient on the back of the cheque. Please do not send in any cash by post. You may pay in cash, NETS or credit card at the Medical Records Unit.
