Request for Medical Report

If you wish to apply for a medical report, we require you to consent to releasing the medical information. If you are below 21 years old, you are required to obtain the consent of your parents or legal guardian. If the person you are applying the medical report for has passed away, a copy of the death certificate and the consent from the deceased's spouse are required.

FAQ

  1. What do I need to bring when collecting the report?   


    You need to bring along your NRIC when collecting your medical report.

    If you are collecting the report on someone else's behalf, you need to produce your NRIC and get an authorisation letter from the patient.

    If the patient is deceased, a copy of the patient's death certificate, supporting documents on proof of relationship and the consent of the patient's spouse/Grant of Administration are required.

    If the patient is a minor, the application is to be made by the patient's parents or legal guardian. A copy of the patient's birth certificate is required.

    Visit us at Medical Reports Unit (4E, Level 4), National Heart Centre Singapore.
    The operation hours are as follows:

    Monday to Friday : 8.00am - 5.00pm 
    Saturday, Sunday and Public Holidays : Closed


  2. How do I make the payment?

    Payment can be made using cash, credit card, cheque or NETS. Cheques should be crossed and made payable to 'National Heart Centre of Singapore Pte Ltd'.

    Please quote the name and NRIC of the patient on the back of the cheque. Please do not send in any cash by post. You may pay in cash, NETS or credit card at the Medical Records Unit.

    Please refer below for the fees applicable for different types of medical reports.

    Fees:

    • Ordinary Medical Report/Completion of Insurance Form by doctor $121.00
    • Specialist Medical Report $222.00
    • Copies of HIDS/Lab results/x-ray reports $12.00
    • Completion of Simple Insurance Form by medical records staff $42.91

  3. How do I submit private medical insurance form (Hospitalisation, Critical Illness and etc.) for doctor’s completion?
    • Please liaise with your respective insurance company or insurance agent to confirm on the appropriate insurance form to submit.

    • Submit the insurance form only after you are discharged.
