Support and provide consultation on heart failure care to healthcare team at the inpatient and outpatient clinical services
Provide health education to enable patients in managing heart failure condition
Provide follow-up care at the outpatient clinic to patients requiring post discharge monitoring and titration of heart failure medications
Provide post discharge telephone follow-up
