Note:

1. RFI Documents are available for collection from MMD at the National Heart centre of Singapore, Materials Management Department, 5 Hospital Drive, Level 1.

2. RFI documents shall be submitted in sealed envelopes or packing and send by hand to the following address:

Monday to Fridays (except public holidays)
8.30am to 6.00pm (except 12-2pm)

3. Terms and Conditions of Proposal

(a) Proposals delivered after the closing date and time for whatever reasons shall not be considered and shall be returned accordingly.
(b) Vendors are taken to have read and shall be bound with full notice and knowledge of the conditions herein.
(c) All expenses and costs incurred in the preparation of the RFI shall be borned by the vendor.
(d) The company, National Heart Centre of Singapore Pte Ltd, reserves the right not to accept the lowest or any proposal without assigning any reason thereof.
