Medical Condition 

Criteria 

Diabetes    

• Age ≥50 and  Type 2 diabetes mellitus (HBA1C 6.5%- 8% within 3  months ) with stable dose(s) of antihyperglycemic medicines
• History of of coronary artery disease [ i.e .,Myocardial infarction, coronary artery bypass graft (CABG), percutaneous coronary intervention (PCI) ], stroke or peripheral arterial disease

Pulmonary Arterial Hypertension

Heart Failure

• NYHA Class II-IV (ordinary activities are limited due to shortness of breath or angina)
• Systolic blood pressure > 100mmHg
• Not on heart transplant list
• Not intent to implant a defibrillator/pacemaker

Coronary Artery Disease

• Age ≥ 55
• Heart rate ≥70 bpm  
• History of heart attack, coronary artery bypass graft (CABG) or percutaneous coronary intervention (PCI)

     OR

• Planned for catheterization/ stent implantation/ ballooning

Ventricular arrhythmia

• Received ICD at least 3 months ago
• On hypertension medication
• Not more than 80 years old
• Not on pacemaker

High Cholesterol Levels

• Total cholesterol concentration > 290 mg/dL (> 7.5 mmol/liter) or a LDL-C concentration > 190 mg/dL (>4.9 mmol/litre)
• Family history of heart attack or raised total cholesterol concentration >290 mg/dL (> 7.5 mmol/liter).
• On statin therapy at stable dose for at least 1 month
